Enhanced Primary Care (EPC) Program
The doctors at Bridge Clinic welcome this Commonwealth Government initiative, introduced in 1999.
Who are these services for?
These services are for older people 75 years or older and indigenous people who are 55 years or older. If you would like to be included in this free service please make an appointment to discuss this with your doctor at Bridge Clinic.
What does the program involve?
It is an annual check up in your own home. Your carer/partner, the EPC nurse and then your doctor look at all the things that affect your day t day well-being. An EPC health assessment covers your physical, social and mental health and looks at whether preventative health care or health education could benefit you. It is a chance for the doctor and you to work together on issues that can improve your health and quality of life.
What will happen during a health assessment?
The EPC nurse will take your blood pressure, look at medicines you take and discuss day-to-day living , asking a range of questions. This information is taken back to your doctor with your permission and then further discussed with you at your follow up appointment. It is also a valuable opportunity to find out what services are available within your local area to assist you with independence and how to access these services. This is a free service with the aim to promote and encourage better health with greater independence and encourages good home safety practices.
